Because of many questions regarding the process for starting a home program or beginning any new service with our organization, we have decided it helpful to explain the general process for new clients. Please know that every client is different and this is only a general path that clients may go on, not a set structured process. Any forms you may need will be at the bottom of the page here. Thank you, and we look forward to serving you. |
|
When you first decide to contact us, we will be asking questions regarding you, your family, and your child. Be prepared for us to ask about the following:
1. Your demographic information (names, phone numbers, addresses).
2. Your child's diagnosis, including when and where the diagnosis was made at.
3. Why you are seeking treatment, what specific issues are you and your family facing, and what are your goals for your child.
4. What other services, including school services, is your child receiving.
5. What questions you have regarding autism, the process of starting, costs, or any relevant matter. |
|
If you find our organization to be a good fit for you, download the following form (Intake Form) and complete it. It asks for a lot of detailed information, which you may not have or remember. Fill out the intake form to the best of your ability. Mail the intake form to us with copies of any documents (IEPs, speech assessments, OT assessments, educational assessments, diagnosis documentations, etc.) and if you have a recent video of your child and positive and negative behaviors, those have been helpful as well. We may request additional documents, but what you can initially gather to return with the intake is fine. Mail all of this information to our office (265 South Harlan Street, Lakewood, CO 80226). |
|
All initial consultations (when within 30 miles of our office) are at no cost to you. Sometimes the intake form and documents are reviewed at this initial consultation, it depends on the family and the situation. The initial consultation is at the families home and is generally about an hour. During this time, we will be able to answer any questions you may have, give immediate suggestions, explain possible ways our organization can assist with you, your family, and your child, and we may conduct additional assessments as well. |
|
After discussing options at the initial consultation, a variety of things can happen. We may start a classic home program, we may provide support to the parents to run their own program, we may provide consulting services to the child's school, and we may provide direct one-to-one services for the child. There are more possibilities, but those are the most common. Please feel free to request services if you are not sure if we provide them. We are great networkers and have lots of resources to forward to parents. |
